Ovarian Conditions

Laparoscopic diagnosis and treatment of ovarian cysts.

The ovaries are the organs that contain the eggs and also produce the hormones estradiol and progesterone. Each month, one ovary prepares an egg which is released at ovulation. The egg matures inside a small sac called a follicle. After ovulation, this follicle normally involutes over the following days. Sometimes, however, these cysts may persist for several weeks — they are then called functional cysts. They rarely cause problems and resolve on their own without medical intervention.

Other cysts may also develop in the ovaries which do not involute and usually require surgical treatment.

The most common types of ovarian cysts are:

Cystadenoma
Endometrioma (chocolate cyst)
Teratoma (dermoid cyst)

Possible symptoms include:

Pelvic pain or discomfort
Menstrual irregularities
Pain during intercourse

Possible complications include:

Cyst rupture causing acute, sudden pain or intra-abdominal bleeding
Ovarian torsion causing acute, sudden pain and ovarian necrosis if not treated promptly
Cyst enlargement

Treatment

Treatment options depending on the case:

Watchful waiting with follow-up
Laparoscopic surgical removal. In premenopausal patients where the cyst appears benign on ultrasound, cystectomy with preservation of the ovary is recommended.
